Please complete our new patient enrollment form to indicate your interest in becoming a patient at Three Rivers Health Center. A member of our staff will follow up with you to gather more information and complete your enrollment.

Our new Patient Portal makes it easier than ever to access your medical records and manage your health.
With our Patient Portal, you can:
Patients will automatically receive an email invitation with a PIN to register for the portal after their appointment, typically within 24 hours.
If you wish to enroll before your appointment, you will need to contact the clinic to obtain a PIN.
